Assessment Coordinator (MA/MSW)
Responsible for conducting mental health assessments, communicating with psychiatrists in determining level of care needed, facilitating admissions and referrals to and from other agencies, conducting pre-certifications and utilization reviews, and fostering positive relationships with all referral sources.
Â Essential JobDuties:
Addresses needs of callers seeking assistance.
Conducts mental health assessments.
Performs pre-certifications in a timely manner.
Performs utilization reviews in a timely manner.Â Communicates with physicians and hospital staff regarding utilization review issues.
Prepares monthly statistical report on assessments.
Communicates feedback from referral sources to Management Team.
Inputs assessment data into the appropriate computer programs upon completion of each assessment.
During periods of critical staffing, will assist clinical staff in patient rounds, help ensure pt safety and comfort, and assist with non-clinical patient care duties under the supervision of a Mental Health Technician, LPN or RN.
